Practicing Medicine in PEI as an International Medical Graduate
PEI has no Practice Ready Assessment route for international medical graduates, unlike some provinces, so registration runs through physiciansapply.ca and one of five accepted LMCC alternatives. Here's exactly what the College of Physicians and Surgeons of PEI checks.
1. One Portal, One Regulator
Every physician registration application to the College of Physicians and Surgeons of PEI (CPSPEI) runs through physiciansapply.ca, the national online application system. There's no separate PEI-only application form.
2. What CPSPEI Actually Reviews
- Your medical credentials and training
- Certifications held
- Certificates of Professional Conduct from every jurisdiction you've practiced in
- Full practice history
- Any other documentation deemed necessary
3. The LMCC Requirement, and Its Alternatives
General registration normally requires being enrolled in the Canadian Medical Register as a Licentiate of the Medical Council of Canada (LMCC). If you don't hold the LMCC, CPSPEI accepts any one of these alternatives:

4. The Route PEI Does Not Offer
Some Canadian provinces run a Practice Ready Assessment (PRA), a supervised clinical assessment pathway for internationally trained physicians who haven't completed a Canadian residency. CPSPEI does not currently have a PRA route. If your plan depends on this pathway, confirm eligibility with a province that does offer it, or focus your PEI application on one of the alternatives above instead.
5. Specialist Registration
For specialists, provisional registration generally requires either certification by the Royal College of Physicians and Surgeons of Canada, or a Council-recognized equivalent body, or completion of a Council-approved clinical assessment process. Post-graduate training must be from a program recognized by the Royal College or approved by CPSPEI Council directly.
6. The Atlantic Registry Advantage, Once You're In
Once you hold General Registration with CPSPEI (or the equivalent in New Brunswick, Nova Scotia, or Newfoundland and Labrador) and your primary practice is in Atlantic Canada, you can apply for the Atlantic Registry, letting you obtain licensure across all four Atlantic provinces without repeating the full application in each one, provided your registration carries no conditions, restrictions, or supervision requirements.
7. Practical Sequence
- Create your account on physiciansapply.ca and identify which LMCC alternative applies to your background.
- Contact CPSPEI directly early, registration review criteria are applied case-by-case, and confirming your specific pathway before submitting saves months.
- Gather Certificates of Professional Conduct from every jurisdiction where you've held a licence, not just your most recent one.
- Once registered, ask about Atlantic Registry eligibility if you might practice or relocate elsewhere in the Maritimes.
